I am participating in a Marlo Fisken Flow Movement Floor Flow Teacher Training ("Training"). I recognize that the Training requires physical exertion that may be strenuous and may cause physical injury, including death, and I am fully aware of the risks and hazards involved. I understand that my participation in this Training is voluntary and that I am responsible for monitoring my own condition throughout the Training and should any unusual symptoms occur, I will cease my participation and inform the staff of the symptoms.
